Description
Dentist – Sign on bonus – 4&1/2 day work week – MDS
Saint Charles, LA.
Four and a half day work week.
Salary - $150k
Four and a half day work week.

fuPROVIDER BENEFITS
15 days vacation for Physicians and Dentists and additional week after 5 years of service
with 30 day notice of approval in advance
10 days vacation for Nurse Practitioners and additional week after 5 years of service
with 30 day notice of approval in advance
12 days of sick time annually
Up to 3 days of Bereavement Leave for immediate family: spouse, mother, father, brother,
sister, son, daughter, grandparents, and grandchildren, in-laws of same category, and aunt
or uncle (sister or brother of mother or father)
Administrative Leave if corporation sends employee to a conference with CEO approval 30
days in advance
$600.00 per month reimbursement for additional insurance coverage
Life Insurance – 2 1/2 times salary – Paid for by facility
Long Term Disability – Paid for by facility – Pays 60% of salary when utilized
Option to Purchase Short Term Disability – Cost is based on salary – Pays 60% of salary
when utilized
$2,500.00 annual CME reimbursement; 5 days annually for CME
$1,500.00 annual reimbursement for dues, licensure, etc.
Malpractice coverage by FTCA; no tail coverage needed if/when provider leaves
401K Plan match dollar for dollar up to 7%
All sites are eligible for NHSC and State Loan repayment programs

POSITION TITLE: DENTIST
IMMEDIATE SUPERVISOR: DENTAL DIRECTOR
REQUIREMENTS AND RESPONSIBILITIES: Has been awarded a D.D.S. or D.M.D.
degree from a dental school accredited by the Commission on Dental Accreditation of the
American Dental Association and holds a dental license in the State of Louisiana, a DEA
Certificate, and a State Controlled Substance Registration. Provides evaluation, diagnosis,
prevention, or treatment, including nonsurgical, surgical, or related procedures, of
diseases, disorders, or conditions of the oral cavity of children and adults. Clinic hours of
operation are Monday & Wednesday from 8am – 5pm, T
