Psychiatric Family Nurse Practitioner

Base Salary Range:                          $100,000 - $120,000 non-bilingual based on experience; bilingual $5,000 additional pay

   Production Bonus:                           $10 per encounter for every encounter greater than 315 per

                                                            month.

   Quality Bonus:                                 $16,000 potential based on quality goals

CME:                                               $1,500.00 annually

License & Association Fees:           Up to $1,500 a year reimbursement                                Health/Dental/Vision/Life:              Organization pays a portion of all

401k - See attachment                     $1 for $1 match on first 3% of income and .50 cents for $1 on 4th and 5th% for a max match at 4% of your income. This is a Safe Harbor Plan so you are 100% vested on the company match immediately.

 Time Off:                                        240 Hours Maximum Per Year (PTO must be used for any CME absence)

                                                            9 Paid Holidays (including your birthday)

                                                            Earn additional extended illness time: caps at 6 weeks

   Loan Repayment:                             NHSC loan eligibility

   Call Rotation:                                  Not required at this time

   Saturday:                                          Not required at this time

   Typical Schedule:                            5 days per week with one late night (total working hours of      40, of that 35 required direct patient contact hours), or alternate agreed upon schedule.

   Production Goals:                            Average daily patient volume >19-25 minimum

   Encounter mix:                                 40% psychotherapy/counseling, 60% medication management.  Must treat ages 5 and up.
