This role is within our Transition To  SNF (TTS)  program.  Optum Complex Care Management provides the Transitions Model of Care for United Healthcare members in a short stay/transitional setting with focus on reduction in 30-day hospital readmissions, reduced ER visits, optimized SNF length of stay, improved completeness and coding accuracy of diagnosis and medical record documentation, increased closure of STAR/HEDIS quality measures, improved completion rates of Advanced Care Planning Directives, and improved patient and family satisfaction.  The Optum practitioner will follow the member until the member is discharged from the TTS program and will conduct a focused visit within 24 business hours of notification, provide a comprehensive visit within 7 days, conduct telephonic or in-person care coordination clinical rounds days 2 through 14 and then every 5 days thereafter.  This is a flexible, autonomous role that creates enormous satisfaction for the NP as you impact the care and comfort of our aging population. All the while, you’ll be building meaningful relationships with the residents and their families and the health care providers who are responsible for their care

Primary Responsibilities:

  • Provide preventative and primary care for patients in assigned in short term rehab facilities and potentially long term care facilities
  • Work with primary care physicians to provide the best care possible
  • Collaborate with the nursing staff and the patients' families
  • Work with the facility's utilization review process
