As a part of our continued growth, we are searching for a new Nurse Practitioner to join our Monarch Cares Wellness Center team.

 
  
Primary Responsibilities:
  • Provide clinical services to members as needed in the Monarch Cares Wellness Centers, member homes, mobile unit and provider offices as needed
  • Educate patients in the diagnosis, treatment and prevention of diseases
  • Counsels patients and families about acute and chronic conditions or concerns
  • Documents items such as: appropriate chief complaint, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment and plan
  • Formulates diagnostic treatment plans and prescribes and administers medications, therapies, and procedures
  • Orders lab and imaging tests to determine and manage an immediate treatment plan and provides advice on follow up
  • Develop and evaluate provider-level standard operating protocols and best clinical practices for Monarch Cares provider staff and vendors
  • Facilitate routine group and individual provider trainings to include new Monarch Cares provider onboarding/training and evaluation efforts
  • Provide day-to-day oversight of clinical quality by performing retrospective chart review of completed charts
  • Quarterly Performance Assessment - Performs audit of all Monarch Cares provider clinical data
  • Act as a daily clinical resource to Monarch Cares vendor provider staff and communicates provider deficiencies and/or concerns to the Monarch Cares Medical Director and vendor medical leadership as appropriate
  • Participate in provider office visits, network provider trainings and events representing Monarch Cares and/or the Monarch Healthcare Risk Adjustment Department
  • Maintains quality of care standards as defined by the practice
