NP / Nurse Practitioner / Oregon / Locum Tenens / NP/PA opportunity for Annual Health Assessments within the Medford, OR area Job

Great opportunity within the Medford, OR area for a Nurse Practitioner or Physician Assistant to perform annual wellness exams working for a private group. 

• Board Certified Nurse Practitioner (NP) or Physician Assistant (PA)
• 2+ yrs exp as NP/PA required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
• Part Time or Full Time hour available - 3-5 days a week
• Mon-Fri 8a-5p
• Perform an annual wellness exam for senior Medicare patients
• Collaborate with Primary Care team to ensure completion of holistic patient care
• This role is a mix of outpatient in-clinic setting, in-home health visits, and occasional telehealth appointments
• Must have active OR medical and drivers license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have AWV exp.
• 8-10 patients per day
• Salary based on experience $125-150K with a sign on bonus and/or relocation assistance and plenty of other great benefits
