Description
The Colorado Coalition for the Homeless seeks two Family Physicians to provide integrated primary health care to individuals at Fort Lyon Health Center (FLHC) located at 30999 CR15, Las Animas, OR Stout Street Health Center (SSHC) located at 2135 Stout Street, Denver. These positions report to the Medical Director of Integrated Health Services.

Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program. Licensed health care providers may earn up to $50,000 toward student loans in exchange for a two-year commitment at an NHSC-approved site through this program.

Essential Job Functions

1.  Provide direct, high-quality, primary medical care to FLHC/SSHC patients; maintains and enhances a current level of knowledge relative to professional practice; and continually strives to improve the quality of care at FLHC/SSHC.
2.  Supervise and collaborate with NP and PA staff by co-signing charts daily, helping direct diagnostic and therapeutic plans in difficult cases, and being available for consultation whenever necessary.
3.  Collaborate with other members of the multidisciplinary health care team to identify and address the complex issues which may adversely impact the health of homeless clients.
4.  Participate as an effective team member in the provision of medical and health care.
5.  Facilitate client participation in self-care by providing information and choices in a non-judgmental, respectful manner.
6.  Take initiative when appropriate to determine ways to deliver health care in a more compassionate, expedited, user-friendly manner.
7.  Maximize community linkages while acting as advocate for patients.
8.  Help to recruit, train, support, and provide evaluation feedback of the medical staff as needed.
9.  Provide clinical precepting opportunities for providers in training as scheduling allows, and as approved by the Medical Director.
10.  Complete all credentialing and privileging requirements of the Coalition.
11.  With the Medical Director and other staff, participate in Quality Improvement and Peer Review activities reviews to provide on-going quality assurance for the clinic.
12.  Maintain Continuing Medical Education and other requirements to keep licensure current.
13.  Maintain EHR core competencies.

Skills
Qualifications Summary
1.  Medical Degree or Doctor of Osteopathy degree from an accredited school of medicine.
2.  Current licensure in the State of Colorado as a physician.
3.  Current DEA licensure.
4.  Completion of an accredited Residency Program in the physician’s specialty.
5.  Board Certified or Board Eligible status in the physician’s specialty preferred.
6.  Bilingual (English/Spanish) preferred.
