Description
The Colorado Coalition for the Homeless seeks two Nurse Practitioners to work collaboratively within a multi-disciplinary Health Care for the Homeless team, and are responsible for providing primary health care to individuals and families at the Fort Lyon Health Center (FLHC) located at 30999 CR15, Las Animas, OR Stout Street Health Center (SSHC) located at 2130 Stout Street, Denver. These positions report to the Medical Director of Stout Street Health Center. The NP works to provide the highest possible quality of care, to optimize available resources, to identify unmet needs and to work with the medical team to improve care.

Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program. Licensed health care providers may earn up to $50,000 toward student loans in exchange for a two-year commitment at an NHSC-approved site through this program.

Essential Job Functions

1.  Provides direct, high-quality, primary medical care to FLHC/SSHC patients; maintains and enhances a current level of knowledge relative to professional practice; and continually strives to improve the quality of care at FLHC/ SSHC.
2.  Collaborates with other members of the multidisciplinary health care team to identify and address the complex issues which may adversely impact the health of homeless clients.
3.  Participates as an effective team member in the provision of medical and health care.
4.  Facilitates client participation in self-care by providing information and choices in a non-judgmental, respectful manner.
5.  Takes initiative when appropriate to determine ways to deliver health care in a more compassionate, expedited, user-friendly manner.
6.  Maximizes community linkages while acting as advocate for patients.
7.  Maintains accurate, legible, and confidential records, and follows HIPAA regulations.
8.  Helps to recruit, train, support, and provide evaluation feedback of the medical staff as needed.
9.  Provides clinical precepting opportunities for providers in training as scheduling allows, and as approved by the Medical Director.
10.  Completes all credentialing and privileging requirements of CCH.
11.  With the Medical Director and other staff, participates in Quality Improvement and Peer Review activities reviews to provide on-going quality assurance for the clinic.  
12.  Maintains Continuing Medical Education and other requirements to keep licensure current.
13.  Maintains EHR core competencies.

Skills
Qualifications Summary
1.  A Master of Science degree in Nursing from a program accredited by the American Nurses Association or the American Association of Nurse Practitioners.
2.  Current registration in the State of Colorado as an Advanced Practice Nurse.
3.  Certification as a Nurse Practitioner from the American Nurse’s Credentialing Center.
4.  Minimum 5 years’ clinical experience
