Community Health Connections is a non-profit, community health center organization providing high-quality, comprehensive outpatient services to families and individuals of all ages. Our health centers, located throughout North Central Massachusetts, include the Fitchburg Community Health Center, Greater Gardner Community Health Center, Leominster Community Health Center, and ACTION Health Services. Here you will find a compassionate, understanding staff and a wide range of services to meet your health care needs. Each of our health centers offers primary medical, full-service dental and behavioral health services. Our Fitchburg center also has an on-site pharmacy. We are committed to caring for all and always accepting new patients. Community Health Connections accepts Mass Health, Health Safety Net, and most commercial health insurance plans. We also help the uninsured enroll in health insurance and offer a Sliding Fee Discount to those who qualify.
