Job Summary: The family practice physician provides health assessment and direct care to patients as part of the provider team providing services to VNA Health Center patients.

Skills Education: Board Certified Physician in Family Practice preferred. Licensed in Illinois required. Knowledge/Skills: Must have the knowledge, skills and ability to provide care to patients. Type/Years Experience: 1-2 years experience preferred as a Family Practice Physician. | ||||||||||||||||||||||
